HIPAA Notice

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

1. Our commitment

HDMeds and the providers and pharmacies you are connected with through our platform are required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

2. How we may use and disclose PHI

• Treatment — sharing your intake and history with the licensed provider who reviews your care, and with partner pharmacies that fill your prescriptions.
• Payment — processing your payments and verifying transactions.
• Health care operations — quality review, safety monitoring, and support.
• As required or permitted by law — public health reporting, health oversight, legal process, and to prevent a serious threat to health or safety.

Other uses and disclosures — including most marketing uses and any sale of PHI — are made only with your written authorization, which you may revoke at any time.

3. Your rights

• Access — inspect and receive a copy of your health records, typically within 30 days.
• Amendment — request corrections to your records.
• Accounting of disclosures — receive a list of certain disclosures we have made.
• Restrictions — request limits on how your PHI is used or shared.
• Confidential communications — ask us to contact you in a specific way or at a specific location.
• Paper copy — receive a paper copy of this notice on request.

4. Breach notification

If a breach of unsecured PHI affects you, we will notify you as required by law without unreasonable delay.

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at hello@hdmeds.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will never be penalized or retaliated against for filing a complaint.

6. Changes to this notice

We reserve the right to change this notice and make the new notice effective for all PHI we maintain. The current version will always be posted on this page with its effective date.
