HDMeds and the providers and pharmacies you are connected with through our platform are required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

2. How we may use and disclose PHI

Other uses and disclosures — including most marketing uses and any sale of PHI — are made only with your written authorization, which you may revoke at any time.

3. Your rights

To exercise any right, email hello@hdmeds.com with the subject “Privacy Request.”

4. Breach notification

If a breach of unsecured PHI affects you, we will notify you as required by law without unreasonable delay.

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at hello@hdmeds.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will never be penalized or retaliated against for filing a complaint.

6. Changes to this notice

We reserve the right to change this notice and make the new notice effective for all PHI we maintain. The current version will always be posted on this page with its effective date.
